MISSION STATEMENT/DEPARTMENTAL PURPOSE:
The purpose of the Aging &
Disability Resource Center is to empower & support older adults, people who have
a disability and their families by serving as a central source of information,
assistance, support and access to community resources.

Information & Assistance: Information
& Assistance Specialists serve persons age 18 and older. I&A
Specialists are Certified Wisconsin Social Workers who are
available to provide accurate and unbiased information on all
aspects of life related to aging or living with a disability.
I&A Specialists assist the public to plan for long and short
term care needs and provide current information about local,
regional, and State resources to eet the care needs of callers
and their friends or family members. I&A Specialists also assist
with community outreach, health promotion, and prevention
services, as well as help eligible persons enroll in long term
programs available through the State of Wisconsin. |

Elderly &
Disability Benefit Specialist Programs: The Elderly Benefit
Specialist provides advocacy and assistance to St. Croix County
residents 60 and older under the supervision of the Coalition of
Wisconsin Aging Groups Elder Law Center. The Elderly Benefit
Specialist provides information about Medicare, Medicaid,
supplemental insurance and all other public benefits, as well as
assistance with grievances and appeals when necessary. The
Disability Benefit Specialist provides services to people ages
18 to 59 with physical disabilities, developmental disabilities,
mental illness and substance use disorders under the direction
of the Program Attorneys at Disability Rights Wisconsin. The
Disability Benefit Specialist provides information about public
and private benefit programs including assistance with
application and appeal procedures. |

Adult
Protective Services: The purpose of the Adult Protective
Services is to provide protection and advocacy to St. Croix
County residents who are elderly or adults at risk. An adult at
risk is an adult, of any age, who has any physical or mental
impairment that restricts his or her ability to care for his or
her needs and who has experienced or is at risk of abuse or
neglect. Abuse includes physical abuse, emotional abuse, sexual
abuse, treatment without consent, unreasonable confinement or
restraint. Other reportable instances include financial
exploitation, neglect and self-neglect. Services provided by the
Adult Protective Services include but are not limited to:
protection, advocacy, outreach, information, intervention,
assessment, short term case management, guardianship and
protective service coordination. |

Senior
Nutrition Services: The Nutrition Program provides a
nutritionally balanced hot meal and place to socialize. We are
funded by Federal, State and County, along with individual
donations. The nutrition sites are open to persons 60 and older
and their spouse regardless of age. Ten senior dining nutrition
sites are located within St. Croix County.

Home-Delivered
Meals & Liquid Supplement: The Home-Delivered Meals program
provides a hot, nutritious meal to persons who are: aged 60 and
over and are unable to prepare adequate meals on their own,
caregivers or spouses who are in need of respite meals,
disabled-co-resident of an individual aged 60 or over who
receives meals. Disabled person under the age of 60 may receive
meals at full cost. Persons are referred by social workers,
doctors, family members or self. Meals are provided on a short
or long term basis. Short term meals are typically provided for
those recovering from surgery or illness. Anyone who receives
this service is asked to contribute toward the cost of the meal.
Contributions are confidential and no one will be denied because
of their inability to pay. In order to ensure satisfaction with
this program and to provide additional nutritional support and
education, the Aging & Disability Resource Center Outreach
Coordinator conducts home visits with individuals served by the
Home-Delivered Meal Program. Liquid supplement is available for
persons unable to maintain a stable weight. |

Volunteer
Medical Transportation Program: The program provides door to
door transportation, to medical appointments only, for persons
who are age 60 and older. The program is designed for those
individuals who need minimal assistance but are no longer able
to drive to medical appointments. Drivers are reimbursed by the
ADRC for the mileage accrued during the transport. Individuals
will be asked to donated, what they are able to afford, to the
ADRC to help defray those reimbursement costs. |

Specialized
Van Transportation: The purpose of the Specialized Van
Transportation program is to provide transportation to person 60
and older and to persons with disabilities, or the spouses of
either. The general public may ride on a “space available”
basis. The vans provide transportation to local senior
centers/nutrition sites, medical, employment, shopping and
social destinations within each community. The vans in three
communities are handicapped accessible. Days and times of van
service vary by community. Requests for service are made by
contacting the senior center location in the community in which
the van service is needed. Requests must be made 24-48 hours in
advance. |

Interfaith
Volunteer Program: Interfaith (Faith in Action) is a
private, non-profit volunteer program which collaborates with
the ADRC to provide volunteer services to those who are age 60
and older. The mission is to assist individuals in ways that
help them retain their independence and remain at home. Services
include: companionship, light housekeeping, transportation,
chore service, respite, telephone reassurance, yard work, and
snow removal. |

Vision
Impairment & Hard of Hearing Support Programs: VIOP provides
information about available resources, large print and taped
materials, and someone to talk to who has a similar visual
impairment. VIOP meets on a monthly basis with transportation
available. Local newspapers and the ADRC News are available on
tape along with applications for talking books. Special outings
are planned as well. A limited number of Closed Caption
Televisions are available for short term loan. |

Family
Caregiver Support Program & Support Groups: The National
Family Caregiver Support Program offers a variety of services to
support caregivers. This includes information and assistance,
caregiver support, education, support groups, respite care
services, peer to peer support, and an annual Caregiver
Conference. The New Richmond and River Falls meetings focus on
memory and cognitive issues. The caregiver group in Hudson is
for caregivers and friends of persons with chronic illness. If
you would like to receive a reminder card please notify the
office. 1st Tuesday in New Richmond from 9:00 a.m. – 10:30 a.m.
at the New Richmond Civic Center, Main Floor Conference Room –
156 E. 1st Street, New Richmond, WI 54017. 2nd Friday in River
Falls from 9:30 a.m. – 11:00 a.m. at the St. Bridget’s Catholic
Church – Westview Room 211 E Division St, River Falls, WI 54022.
3rd Thursday in Hudson from 1:00 p.m. - 2:30 p.m. at the St.

DayAway
Club/Adult Day Respite: The Adult Day Respite Program is a
public service, social model respite care program. The 6 hours
structured day with activities, morning snack and noon meal is
designed for those who are frail or have cognitive problems
related to dementia. The Adult Day staff and volunteers provide
a stimulating, healthy, caring, safe environment and meaningful
activities. Activities include: physical exercise, socializing,
games, puzzles, card games, crafts and artistic projects, music,
baking, movies, and occasional outings. A morning snack, and
hot, nutritious lunch is provided each day. There is a daily fee
for the program. The program is offered four days a week at
various locations in St. Croix County. An in-home assessment and
physician’s form are completed prior to acceptance to the
program. Transportation may be possible. |
