Register in Probate - Testamentary Trust Termination TRUST TERMINATION When it is determined that a testamentary trust can be terminated, please use the guideline available through this link. The guideline explains the steps to take in terminating the testamentary trust, lists the forms required, and provides general information to the trustee. A Final Trustee Account must be filed. Form PR-1937 can be found at www.wicourts.gov, under forms, circuit court; or call the probate office to request the form. The probate office can also provide the documents to terminate the testamentary trust. Please call the probate office for the forms if you are unable to access them here.
